Our Metrics working group are developing an industry wide, universal measure for error. The concept is to develop a system that measures the “Error Frequency Rate” with a monthly rolling score and annual target. It will work in a similar way to the Accident Frequency Rate which indicates the number of injuries per 1,000,000 employee-hours worked. The team face a few challenges to define this new system, and the questions being asked include: what will the error frequency rate measure? Absolute number of errors?
